Quante volte abbiamo parlato di by pass gastrico? È una tecnica chirurgica che permette di intervenire sullo stomaco riduce l’assorbimento del cibo che transita nelle vie gastro-intestinali. Questo era prima, ora c’è una nuova soluzione. Si tratta di Endobarrier, un dispositivo (un tubo di materiale plastico), che viene introdotto nel primo tratto del duodeno, e che costituisce appunto una barriera interna tra il cibo e la mucosa intestinale.
Questa metodica ha molti vantaggi rispetto alle altre tecniche di chirurgia dell’obesità. Prima di tutto è una procedura totalmente endoscopica: in sostanza il dispositivo viene introdotto in anestesia totale per via naturale, dalla bocca e attraverso lo stomaco, e il paziente può lasciare l’ospedale entro 48/72 ore. In secondo luogo si tratta di un intervento reversibile: l’obiettivo è di lasciare nell’intestino per un anno il dispositivo, che però può essere rimosso in qualsiasi momento. Infine è una tecnica meno costosa, il che non guasta, rispetto alla chirurgia tradizionale per l’obesità, per la ridotta degenza e il risparmio sui farmaci.
Ha spiegato al Corriere della Sera Andrea Formiga, responsabile dell’unità chirurgica Capitanio dell’Istituto Auxologico Italiano, tra i primi ad aver sperimentato l’Endobarrier. Questa tecnica in Europa, soprattutto in Italia, è una novità ma è già stata utilizzata con grande successo, mentre negli Stati Uniti non è stata ancora autorizzata dall’Fda. Con questo intervento si ottiene un calo del 30 percento del peso, mentre per i valori metabolici si può registrare una riduzione del 7 percento della glicemia e i due punti dell’emoglobina glicata. Come deve essere la dieta del paziente? Ipocalorica e soprattutto con cibo ben cotto, ben masticato e cibo frammentato.
